3. Required information: personal application, copy of ID card, copy of household registration book, hospitalization invoice, approval form for reimbursement of new rural cooperative medical system or medical insurance, discharge summary (record) and other relevant information.

With the continuous advancement of deepening medical reform, China has basically formed a multi-level medical security system with basic medical insurance for urban and rural residents, basic medical insurance for employees, serious illness insurance and medical assistance as the mainstay, supplemented by commercial health insurance.
