First, the physical examination time
Monday to Friday 13:30- 15:00, please come on time, and rest on Saturday, Sunday and national holidays.
Second, the handling of materials
Shanghai employees (16 years old or older) shall submit the following documents to jiuting town Community Health Service Center when receiving health examination. You can apply for free if the materials are complete:
1. Copy of the business license and affix the official seal of the company.
If you don't have a business license, you can show the following materials: organization code certificate, private non-enterprise registration certificate, a copy of the public notice card for temporary filing of convenience restaurants, and a copy of the public notice card for temporary operation of food vendors, which must be stamped with the official seal of the employer or operator.
2. Employment certificate or proposed employment certificate
The form can be labor contract, work permit, employment agreement, etc.
Note: The above employment certificate is for reference only, and A4 paper must be used for printing.
3, employees' valid identity documents and copies (front and back)
Original and photocopy (front and back) must be brought.
Note: Those who can't submit the above certification materials and insist on physical examination will be charged relevant fees according to the regulations after passing the examination. The charging standard is as follows: D Food practitioner certificate: 139 yuan/person, which can be used in food, cosmetics, public places, drinking water and other industries.
ABC certificate of other employees 105 yuan/person, which can be used in cosmetics, public places and drinking water industries, but not in food.

One day before physical examination: avoid alcohol, have a light diet and pay attention to rest.

V. Inquiries about health certificates
1. After the physical examination, you can log in to 10 working day and click on the health certificate to download and inquire.
For those who fail the physical examination, our hospital has full-time staff to call for re-examination (please fill in the correct mobile phone number and keep the phone open); Those who have not re-examined for more than three months will be disqualified and need to re-examine.
2. If you can't find a health certificate, you can come to the physical examination department for consultation with your ID card from Monday to Friday 13: 30- 15: 00 (closed on Saturdays, Sundays and holidays).
