1. If you need a physical examination, go to the information desk to get a physical examination guide, read it carefully and fill in your name, gender, age and job category, with clear handwriting.
2. Pay the invoice at the payment window with the medical examination instruction sheet and get the portrait.
3. Go to the relevant departments for physical examination according to the physical examination instruction. Because the physical examination and certification are all managed by computer, we remind the physical examiners to check every item. Anyone who is short of goods can't get a health certificate. If you have specific difficulties, please consult a doctor or director.

If the physical examination results meet the requirements for issuing a health certificate, after passing the training of health laws and regulations organized by health supervision institutions, I will take my one-inch bareheaded color photo to the health supervision office of the health bureau under my jurisdiction to apply for a health certificate. If the physical examination results do not meet the conditions for issuing a health certificate, a health certificate shall not be issued.

Tenth health supervision institutions at all levels should formulate preventive health examination work plan.

Thirteenth health inspection units shall submit the original records and health inspection results of the inspected personnel to the health supervision institutions.
Fourteenth health supervision institutions shall, according to the results of health examination, issue health certificates to those who pass the preventive health examination. Put forward opinions on handling unqualified personnel and supervise their implementation. At the same time, the original materials should be submitted to the inspected unit or the designated filing unit for filing.
