Health certificate processing flow:
First, according to the list of personnel determined by the health supervision institution, go to the health examination unit for preventive health examination, and uniformly use the "People's Republic of China (PRC) Preventive Health Examination Form" formulated by the Ministry of Health.
Second, conduct a physical examination before handling the health certificate.
1, 2 ID cards and one-inch color photos of individuals bareheaded, pay the medical examination fee at the toll booth, get the medical examination form, paste the photos (the other one is a health certificate), and fill in the basic information such as name and gender.
