According to the provisions of the Law on the Prevention and Control of Occupational Diseases, the employer shall establish occupational health monitoring files for the workers, and the contents of the files shall include: workers' occupational history, occupational disease exposure history, occupational health examination results, occupational disease diagnosis and treatment, and other relevant personal health information.

Requirements for establishing occupational health monitoring files:
1. Article 20 of the Law of People's Republic of China (PRC) on the Prevention and Control of Occupational Diseases stipulates that the employer shall establish and improve occupational health files and workers' health monitoring files.
2. Article 36 of the Law of People's Republic of China (PRC) on the Prevention and Control of Occupational Diseases stipulates that the employer shall establish occupational health monitoring files for the workers and keep them properly within the prescribed time limit; When leaving the employer, the laborer has the right to ask for a copy of his occupational health monitoring file, and the employer shall provide it truthfully and free of charge, and sign the copy provided.

1, preparation requirements:
The employing unit shall establish occupational health monitoring files in accordance with the relevant laws, regulations and requirements for occupational disease prevention and control, and the occupational health management department shall conduct comprehensive management, compile a summary table of occupational health monitoring files, formulate a registration management system for consulting, copying and informing, and designate a special person to manage them.
2, one person, one file:
"Occupational Health Surveillance Archives" should be filed by one person, with a special file cover and placed in a special filing cabinet. Among them, the chest radiograph can be filed according to the actual situation of the employer, and can be put into the occupational health monitoring file for one person and one file, or it can be kept in a centralized way, but the file number and name must be indicated when it is kept in a centralized way, and a catalogue must be compiled.
3. Time of establishment:
When workers enter the employing unit to engage in operations that are exposed to occupational hazards (including transferring to posts that are exposed to occupational hazards) and operations with special health requirements, occupational health monitoring files shall be established.
