In addition to the corresponding environmental assessment report, pollutant discharge permit and fire control acceptance, the following information shall also be provided:
A, employee physical examination report, all employees involved in the pollution process in daily work need physical examination, and provide physical examination report (no occupational disease);
B. On-site inspection report. The above-mentioned workshop producing pollution process needs on-site inspection and provides inspection report (pollutants are within the allowable range);
C, cranes, forklifts, welding machines and other special equipment and their operators, equipment must have a certificate of approval, personnel must have an operation certificate;
D, engaged in dangerous chemicals (flammable, explosive, etc. ) operation requires a hazardous chemicals business license;
E, when there is a need for aerial work, lightning protection detection is required;
F. If there is a power distribution room, insulating gloves, shoes and other items are needed;
G. If the company has a canteen, it needs a food business license and the chef needs a health certificate.
