1.From 0: 00 on May 20201/day, the online health declaration service platform "Health Declaration Portal" in COVID-19, Henan Province was closed and online declaration was suspended. For those who have successfully submitted their application requirements before May 1 1, grassroots medical and health institutions should continue to do a good job in on-site screening and evaluation, and provide free services. From May 16, the online service platform "responsible doctor entrance" was closed, and all grassroots medical and health institutions in the province no longer accepted online and offline services of health declaration certificates.
2. After the suspension of the health declaration certificate service, passengers can apply for the "Health Code" of Henan Province on Alipay platform, or apply for the national unified "Epidemic Prevention Health Information Code" on Alipay and WeChat platforms. If they can't apply, they can apply for a health pass in the community neighborhood Committee.
Three, grassroots medical and health institutions should keep the online and offline application materials, to ensure the information security of the applicant.
