According to the relevant provisions of the provincial and municipal headquarters, in order to do a good job in the management and service of personnel leaving Hubei in high-tech zones, relevant matters are hereby notified as follows:

First, individual application

Health status. The applicant must obtain the green code of "Hubei Health Code", or the green code of "Health Code" in other provinces, or the health monitoring certificate of the place of departure.

Proof of receipt. The applicant has obtained the certificate that the unit or community where the destination is located agrees to accept it.

Application materials. If the applicant has a suitable work unit, he shall apply to his starting unit, and the application materials shall be unified to the community where the applicant is located after the preliminary examination of the unit; If there is no work unit, apply to the local community.

The application materials mainly include:

1. Proof of destination receipt;

3. The health code of myself and the attendant or the health monitoring certificate of the place of departure or the official seal of the unit).

Two. Verification and arrival

Verification. Arrive at the traffic control point, scan the green codes of "Hubei Health Code" one by one or check the green codes of "Hubei Health Code" from other provinces, show the original application form and personal ID card of Yichang High-tech Zone personnel leaving Hubei, and go through the travel registration after my ID card matches and my temperature is normal.

Arrive. After arriving at their destination, people leaving Hubei must immediately report to the communities and units at the place of departure, and strictly abide by the relevant provisions of local epidemic prevention and control management.

Third, the access mode.

In the case of implementing epidemic prevention and control and safety measures, the applicant can leave Hubei safely and orderly by self-driving or "point-to-point, one-stop" chartered travel. This notice shall be implemented as of March 16, 2020.
