Legal analysis: materials that individuals need to prepare health certificates: (1) the original ID card of the medical examiner. (2) A copy of the employer's hygiene license and stamped with a fresh stamp, a copy of the business license and stamped with a fresh stamp, and an employment certificate issued by the employer and stamped with a fresh stamp, both of which shall be consistent with the name of the employer. (3) If the employing unit that has not yet obtained the official seal organizes the employee's physical examination, in addition to providing the above information, it also needs to produce the original hygiene license or business license.
Legal basis: Article 45 of the Food Safety Law of People's Republic of China (PRC), food producers and business operators shall establish and implement the health management system for employees. Persons suffering from diseases that hinder food safety as stipulated by the administrative department of health of the State Council shall not engage in direct contact with imported food. Food production and marketing personnel engaged in direct contact with imported food shall undergo annual health examination and obtain health certificates before taking up their posts.
