(4) The second to third pages are health examination records, including blood pressure, pulse, development, nutrition, skin, lymph nodes, eye vision, corrected vision, color discrimination, otolaryngology, thyroid, lung, heart, abdomen, spine, limbs, urogenital system, anus, nervous system, electrocardiogram, chest X-ray examination, laboratory examination and other examination items.
(5) The contents of pages four to five are the same as those of pages two to three;
