1 September 29th, 998 to1year1October 60th, the 7th World Health Organization/International Hypertension Union Hypertension Conference was held in Japan.
At this meeting, new guidelines for the treatment of hypertension, new diagnostic criteria for hypertension and a comprehensive prevention and treatment plan for hypertension were determined.
Defined as: systolic blood pressure is greater than or equal to140mmhg, diastolic blood pressure is greater than or equal to 90mmhg, and no antihypertensive drugs are taken; I used to have a history of hypertension, and now I use antihypertensive drugs. Although my blood pressure has not reached the above level, I should also be diagnosed with hypertension.
China unified the standard of blood pressure rise with the standard set by the World Health Organization.
Detailed standard
(Prepared by WHO 1978 for reference)
That is, after three times of examination and verification, according to the level of blood pressure, it is divided into normal blood pressure, borderline hypertension and diagnosed hypertension.
(1) Normal blood pressure: systolic blood pressure 18.7kPa( 140mmHg) or less, and diastolic blood pressure 12.0kPa(90mmHg) or less, which is not hypotension, should be regarded as normal blood pressure.
(2) Critical hypertension: systolic blood pressure18.8 ~ 210.2kpa (14159mmhg) diastolic blood pressure12.1~/.
(3) Diagnosed hypertension: systolic blood pressure reached or exceeded 2 1.3kPa( 160mmHg), and diastolic blood pressure reached or exceeded 12.7kPa(95mmHg).
China
The Guidelines for the Prevention and Treatment of Hypertension in China refer to the Guidelines for Hypertension in 1999WHO/ISH, and classify the blood pressure of adults over 18 according to different levels:
Ideal blood pressure (
Normal blood pressure (
Normal high value (130—139/85—89mmhg),
1 grade hypertension (140-159/90-99mmhg),
Grade 2 hypertension (160—179/100—109 mmhg),
Grade 3 hypertension (≥180/110mmhg),
Patients with isolated systolic hypertension (systolic blood pressure ≥ 140/ diastolic blood pressure < 90mmHg), systolic blood pressure and diastolic blood pressure belonging to different levels should be classified according to the higher level of the two.
Latest hypertension standard
In 2004, the guidelines for prevention and treatment of hypertension in China basically adopted the diagnostic criteria of hypertension of the World Health Organization.
The high value of120 ~139/80 ~ 89 mmhg is the result of epidemiological data analysis in China.
Those whose blood pressure is within this range should seriously change their lifestyle and take early preventive measures to avoid hypertension.
(See table below) Systolic blood pressure category
Diastolic pressure (mmHg)
Normal blood pressure
Hypertension ≥ 140
Note: If systolic blood pressure and diastolic blood pressure belong to different grades, the higher grade shall prevail.
United States of America
The seventh report of the National Joint Committee on Prevention, Detection, Evaluation and Treatment of Hypertension in the United States divides blood pressure levels into four categories:
Normal blood pressure (≤ 120/80mmHg),
Prehypertension (120-139/80-89mmhg),
1 stage hypertension (140-159/90-99mmhg),
Stage 2 hypertension (≥ 160/ 100mmHg).
The obvious change compared with before is that the original normal blood pressure 120/80mmHg is defined as "prehypertension", while the normal standard blood pressure is reduced to 1 15/75mmHg.
On the one hand, lowering the standard of normal blood pressure reminds people to know, treat and control early, but on the other hand, the incidence and prevalence of hypertension will also increase greatly, and the task of preventing and treating hypertension is even more arduous.
Prehypertension requires healthy lifestyle intervention to prevent cardiovascular and cerebrovascular diseases.
In addition, for people aged 40-70 years, the blood pressure level starts from 1 15/75mmHg, and the risk of cardiovascular and cerebrovascular diseases increases by 1 times with an increase of 20/1mmhg.
For patients over 50 years old, systolic blood pressure is more important than diastolic blood pressure.
Therefore, the systolic blood pressure must be reduced to below 130- 140 mmhg.
Europe
In the 2003 European Guidelines for the Prevention and Treatment of Congestion, the diagnostic criteria of hypertension with different blood pressure measurement methods were put forward, namely:
Clinical blood pressure is 140/90mmHg,
The 24-hour ambulatory blood pressure was 125/80mmHg,
Home self-measured blood pressure 135/85mmHg.
The goal of antihypertensive therapy is to reduce blood pressure below 140/90mmHg; Diabetic patients should be below 130/80mmHg.
Japan
The normal blood pressure of Japanese hypertension conference is 130/85mmHg, and the ideal blood pressure is 120/80mmHg.
The society has also formulated the normal blood pressure of the elderly:
60-69 years old 140/90mmHg,
70-79 years old
80-89 years old
Why do countries differ slightly in the formulation of hypertension standards? The new regulations of European and American hypertension standards only increase the so-called "prehypertension" value between "normal blood pressure" and "hypertension", or partially adjust the scope of "normal blood pressure", without modifying the previous WHO clinical diagnosis standard of hypertension 140/90.
The purpose of these new regulations is to start the preventive measures of hypertension in advance, so as to strengthen medical intervention, which is of great significance for controlling blood pressure in patients with chronic diseases such as diabetes.
The term "prehypertension" in the United States is not suitable for China's national conditions for two reasons: First, China's medical and health resources are relatively insufficient, and the term "prehypertension" may cause some doctors to "overexert" in treatment, resulting in greater medical burden; Second, I am afraid that some patients will be too nervous when they hear "hypertension" and interfere with normal treatment.
At the Asia-Pacific conference on prevention and treatment of hypertension held some time ago, experts from all countries agreed that China's blood pressure standard is the most scientific and most suitable for Asian countries.
