What documents does a five-year-old child need to apply for a health certificate?

I learned from the health and epidemic prevention department that there are generally the following procedures for handling health certificates:

1. Take your ID card to the health certificate office to get the health check list, fill it out clearly one by one according to the requirements on the form, and post my photo.

2. Pay at the toll station with the payment notice issued by the health certificate handling office, and return the notice to the registration office.

3. Participate in health knowledge training.

4. Health examination, blood drawing, anal examination and chest X-ray examination.

5. Handling of health certificate: One week after the physical examination, bring two one-inch photos of yourself and take the training certificate to the health certificate handling office to get the certificate.

The health certificate is not handled in the hospital.

Instead, apply for a health certificate at the local health and epidemic prevention station (now renamed the Center for Disease Control and Prevention), and then they will ask you to fill out the form and provide your personal photo and ID card. Then they ask you to go to the designated place and have a physical examination of the designated items. They usually go to some of their own departments for examination, and don't think about the hospital you are going to. Generally, if there is no infectious disease in the physical examination, then they will give you a health certificate.
