Second, the application and acceptance procedures for occupational disease diagnosis
1, workers can apply for occupational disease diagnosis to the occupational disease diagnosis institution where the employer is located or where I live, including the county (district) where the employer is located or where I live.
Any occupational disease diagnosis institution in the city or province where the county is located is applicable.
2. Occupational disease diagnosis shall be carried out by the employee or the employing unit (hereinafter referred to as the party) and its agent, and an application form for occupational disease diagnosis shall be filled in.
And submit the following materials required for occupational disease diagnosis:
(a) a certificate of health damage and a complete current medical history
(2) professional history certificate;
(3) Copies of occupational health monitoring files;
(four) the results of occupational health examination over the years;

Among the submitted occupational disease diagnosis materials, the occupational history certificate and labor relationship certificate should be original, and other materials can be provided with copies, but the materials provided should be marked as "the same as the original".
And signed by the provider. All information shall be kept permanently by the occupational disease diagnosis institution and shall not be returned or lent. The parties concerned shall keep copies or documents for themselves.
When an agent applies for occupational disease diagnosis, it shall submit the power of attorney of the party concerned and show the original ID card of the agent.
3. When receiving the application for occupational disease diagnosis and related materials, the occupational disease diagnosis institution shall issue a receipt for the application for occupational disease diagnosis.
4. The occupational disease diagnosis institution shall complete the written examination within 5 working days from the date of receiving the application materials.
If the materials are complete and conform to the statutory form, a notice of acceptance of occupational disease diagnosis shall be issued to the parties concerned, and the applicant shall pay the occupational disease diagnosis fee.
If the materials are incomplete or do not conform to the statutory form, the applicant shall be issued a notice of correction of the application materials for occupational disease diagnosis, and the applicant shall submit it within 15 days from the date of receiving the notice. If it is not submitted within the time limit, it shall be deemed as not provided. However, if a worker applies for occupational disease diagnosis, the employer shall provide supplementary materials, which shall be submitted within 15 days from the date of receiving the notice. If it is not submitted within the time limit, it shall bear corresponding legal responsibilities. Occupational disease diagnosis institutions can handle the acceptance procedures according to the relevant certification materials provided by the applicant and report to the health administrative department where the employer is located.
5. The occupational disease diagnosis institution may reject the following cases and issue a notice of rejection of occupational disease diagnosis: it does not belong to the catalogue of occupational diseases; No contact history with occupational hazards; No abnormality was found in the health examination; It does not belong to the occupational disease diagnosis items and the scope of authority approved by the approval certificate; Diagnostic institutions have made occupational disease diagnosis, and there is no new evidence; The applicant fails to submit supplementary materials within 15 days, and fails to explain the justified reasons in writing.
6, occupational disease diagnosis institutions within 30 working days from the date of acceptance of the organization diagnosis, there are special circumstances need to be extended, explain the reasons and inform the applicant.
