Second, the physical examination process
(1) Fill in the health declaration form
This is a new project after SARS. Filling in personal information on the form is very simple.
(2) Take the form of physical examination.
Fill in the health declaration form and get it to reception window 2, stating the purpose of going abroad (studying abroad) and the purpose of going abroad (the United States). If the school has special inspection items (such as PPD), it should also be explained. If not, press the default item for physical examination. Take a physical examination form, there are several forms in it, and you need to fill in personal information, both in Chinese and English.
(3) Payment
After filling out the physical examination form, you can charge at the payment window inside. The examination items include: X-ray examination, electrocardiogram, blood test, general examination and immunization (two injections by default, MMR and diphtheria and tetanus).
(4) Physical examination items
X-ray examination and blood test are on the first floor, and X-ray examination, electrocardiogram, general examination and injection are on the second floor.
Urine test: Drinking more water before physical examination is good for urine test results.
Venous blood test, can not be empty stomach, mainly for AIDS, syphilis and other infectious diseases.
3. After physical examination: Go back to the lobby on the first floor, put the physical examination form, immunization certificate, physical examination form and a 2-inch photo issued by the school in the envelope and hand them in to 1 reception window. You will be given a travel medicine box with a thermometer and common medicines.
