The process of handling health certificate is as follows:
1, physical examination is the first;
(1) Bring your ID card and two one-inch color photos without a hat, pay the medical examination fee at the toll booth, get the medical examination form, paste the photos (the other one is for health certificate), and fill in the basic information such as name and gender;
(2) Laboratory blood collection and stool anal swab test;
(3) Routine physical examination of internal medicine and surgery;
(4) chest x-ray;
(5) Complete all physical examination items.
2. If the physical examination results meet the conditions for issuing health certificates, and after passing the training on health laws and regulations organized by health supervision institutions, I will take my one-inch bareheaded color photo to the health supervision office of the health bureau under my jurisdiction to apply for health certificates. If the physical examination results do not meet the conditions for issuing a health certificate, a health certificate shall not be issued.
Health certificate refers to the certificate of preventive health examination, which proves that the examinee has the health quality stipulated by the industry. Health certificate mainly involves six diseases in five industries, which largely protects the health of employees and clients.
Health certificate refers to the pre-post, pre-post and age-appropriate health examination for personnel engaged in food and drinking water production and management, personnel directly engaged in cosmetics production, personnel who directly serve customers in public places, hazardous workers, radiation workers and school students according to the provisions of national health laws and regulations.
Article 7 of the Regulations on Hygiene Management in Public Places * * * Personnel who directly serve customers in public places must hold a Health Certificate before they can engage in their own jobs. Persons suffering from dysentery, typhoid fever, viral hepatitis, active tuberculosis, purulent or exudative skin diseases and other diseases that hinder public health shall not engage in direct customer service before they are cured.
Twelfth health and epidemic prevention institutions of public health supervision responsibilities:
(a) health monitoring and health technical guidance in public places;
(two) to supervise the health examination of employees and guide the relevant departments to carry out health knowledge education and training for employees.
