How to recuperate after myocarditis?
Viral myocarditis is a disease caused by many viruses, such as focal or diffuse myocardial cell degeneration, necrosis, interstitial inflammatory cell infiltration and fiber exudation, which leads to myocardial injury, cardiac insufficiency or arrhythmia. This disease can cause pericarditis and endocarditis, which is called viral myocarditis pericarditis or panpericarditis. Its clinical manifestations vary greatly. Mild patients may have no obvious conscious symptoms, but they are occasionally found during physical examination, or have mild discomfort symptoms, such as fatigue, sweating, palpitation, dizziness, chest tightness, shortness of breath or shortness of breath. The first heart sound at the apex of auscultation is dull, and premature beats and ST-T changes can be seen in ECG examination. In severe cases, acute onset may occur, such as obvious fatigue, palpitation, shortness of breath, discomfort or pain in precordial area, nausea, vomiting, abdominal pain, etc. During the examination, cardiac insufficiency such as slight enlargement, arrhythmia and galloping rhythm can be seen. Extremely severe patients may develop heart failure and cardiogenic shock within a few hours to 1-2 days after onset. If they can't get timely and correct rescue, they may die easily. If the disease persists, it will develop into chronic myocarditis, mostly recurrent heart failure or severe arrhythmia, and the heart will expand progressively. It can develop into dilated cardiomyopathy and eventually die without treatment. In addition, at the same time of onset or within 1-3 weeks before onset, there is a history of virus infection in respiratory tract, intestine and other systems, and myocardial enzymes are often increased and anti-myocardial antibodies are positive during the course of disease. Traditional Chinese medicine does not have the name "viral myocarditis". Judging from its onset characteristics and clinical manifestations, it seems to be included in many diseases recorded in classic medical books, such as typhoid fever, sudden death, watery heart, hyperhidrosis and so on. For example, "Treatise on Febrile Diseases" pointed out: "The typhoid pulse is transmitted from generation to generation, and it is frightening, mainly with the fried licorice soup." These are not listed one by one. All these have laid a foundation for us to understand and treat viral myocarditis by using the theory and method of traditional Chinese medicine today. Common symptoms: 1. Fever, sore throat, cough, vomiting, diarrhea, fatigue or muscle soreness occurred more than 1-3 weeks before the symptoms of primary virus infection appeared. 2. Myocarditis is characterized by palpitation, chest tightness, shortness of breath, fatigue, precordial pain, even shortness of breath, purple mass, edema, or syncope and convulsion. Commonly used Chinese patent medicines belong to the categories of febrile diseases, palpitations, paralysis and chest pain. Can be used according to different stages. (1) acute phase 1. Wind-heat type: Symptoms include fever, cough, chest tightness, palpitation, yellow stool, red tongue tip and rapid pulse. Treatment: expelling wind, clearing heat and detoxifying. Can be taken: Yinqiao Jiedu Pill, Banlangen Granule, Qingre Jiedu Oral Liquid, etc. 2. Damp-heat type: symptoms include low or little body heat, bitter and sticky mouth, vomiting or diarrhea, anorexia, abdominal distension, red urine, yellow and greasy fur and rapid pulse. Treatment: clearing heat and eliminating dampness. Can be taken: Ganlu Duqing Dan, Jiawei Xianglian Pill, Gegenqin Lianpian, etc. (2) Recovery period and extension period: 1. Deficiency of both Qi and Yin: Symptoms include palpitation, shortness of breath or low fever, especially in the afternoon, spontaneous sweating, chest tightness or pain, upset and insomnia, and rapid pulse or stagnation. Treatment: Yiqi Yangyin Ningxin. Can be taken: Shengmai Yin, Bai Zi Yangxin Pill, Ginseng Guipi Pill, Ginseng Yang Rong Pill, etc. 2. Crazy blood clot. Symptoms include chest tightness, palpitation, tingling in the precordial region, or exposure of the blue veins in the neck, blood stasis on the dark side of the tongue, and thin or tight pulse. Treatment: regulating qi, promoting blood circulation, relieving pain and calming the heart. Can be taken: Bazhen Pill, Shixiao Powder, Qi Dan Tablet, Yufeng Ningxin Tablet, etc. 3. Internal resistance of phlegm-dampness: symptoms include obesity, chest tightness, dizziness, palpitation, anorexia, abdominal distension, white and greasy fur, slippery or knotted pulse. Treatment: invigorating spleen and eliminating dampness. Can be taken: Chen Er Pill, Xiangsha Junzi Pill, etc. (3) Patients from the chronic stage to the present stage generally have serious pathological changes, and most of them are accompanied by diseases such as heart and spleen, heart and kidney, yin and yang deficiency, etc. It is characterized by palpitation, shortness of breath, wheezing when moving, cold limbs, spontaneous sweating fatigue, edema, dull complexion, dark tongue, white fur and pulse knot. Treatment: supporting yang and benefiting qi, nourishing yin and calming the nerves. Can be taken: Jisheng Shenqi Pill, Tianwang Buxin Pill, Ginseng Guipi Pill, Shen Rong Anshen Pill, Feng Ping Anxin Capsule, etc. Commonly used western medicine 1. Antivirus Up to now, there is no specific antiviral drug. At present, amantadine, moline biguanide and interferon are commonly used for antivirus. Some people think that when the disease is diagnosed, the virus damage to the heart has already formed, and the use of current antiviral drugs can not completely prevent the development of the disease, and there is the possibility of damage to the body tissue, so it is not recommended to use it. Some people think that bacterial infection and viral infection often coexist or occur successively, so it is worth considering to control bacterial infection in time with appropriate antibiotics. 2. Treatment with a large amount of vitamin C, coenzyme A and polarized liquid (glucose plus insulin plus potassium chloride) can promote myocardial repair, reduce autoimmune reaction and help prevent the development of the disease. 3. Symptomatic treatment For those with severe arrhythmia, cardiac insufficiency, hypotension or shock, consider using hormones as soon as possible, taking prednisone orally for 40-60mg/ day, and reducing the dosage for 4-6 weeks after remission. However, the use of hormones is still controversial. People with arrhythmia can choose corresponding antiarrhythmic drugs according to different situations. In shock, dopamine can be injected intravenously. Traditional Chinese medicine has certain advantages in treating viral myocarditis, and you can find an experienced pediatrician of traditional Chinese medicine for diagnosis and treatment.
