How to fill in the registration form of health status of candidates in senior high school entrance examination

Good health, no medical history, no infection during the epidemic, and normal nucleic acid detection. There is no history of going abroad in the recent or recent year, and there is no family history.

Candidates are required to monitor their physical health every day, measure their body temperature and truthfully record their personal health information. All candidates who take the exam should take daily temperature measurement on their own from 14 days before the exam, and fill in the Registration Form for Self-monitoring of Candidates' Temperature.

Candidates need to truthfully fill in their own health status before the exam, and fill out the Report Form of Health Status of Candidates of Jiangsu Primary and Secondary School Teacher Qualification Examination in the Second Half of 2020, referred to as the Health Report Form.
